Registration

To secure your child’s enrollment, you must complete, sign, and return the following forms:

• Registration/Emergency Contact Form

• Enrollment Application Form

• Classroom-Child Information Form

• Child Health Form/Including all immunizations (or equivalent provided by doctor)

• Parent Handbook Agreement Form

Registration Fee

A non-refundable registration fee of $100.00 is due upon registration for new enrollments.  New enrollments are also required to pay the first week of tuition.  Should you choose not to attend SBLC, your payment is non-refundable.
